I imagine this is covered in "These Are The Voyages vol 1", which I don't have. I expect it's an "uh-oh ... this is what the script gave us, and it's too late to fix it." So if I'm pointing to anyone, it'd be the showrunner (Gene Coon) and the script supervisor (I think that was John D.F. Black then, but not sure). Meanwhile, continuity was always a problem and it was getting late in the season and people were tired.

So ... seven in the transporter is annoying, but the Fallout Shelter sign on a building in the 1930s isn't? :-D That's the other thing that gets pointed out most. Memory Alpha has a nice list of continuity errors for the episode.
